Guide - Ƶ Applicant Supplemental Forms

This instruction guide explains how to fill out the Ƶ Applicant Supplemental Forms. The guide is designed for international students who wish to apply to study at Ƶ.

If you are an incoming Exchange, or a Sponsored Visiting student from a Partner Institution/Organization, please contact Ƶ Admissions for a specialized application package for your institution/organization.

To download and open these PDF forms, you need to have installed on your computer. Please note that these forms may not work well on mobile devices such as iPads, tablets or mobile phones.

The forms are fillable PDF files. Please fill them in electronically and sign.
The student's signature MUST match the signature on the passport.

University Transfer: Target University & majorIf this option appears, your selected program of study has transfer arrangements with another institution(s). Please include a target university and a major. If you are not interested in transferring, please type "I do not wish to transfer" in this field.
I intend to complete a version of this program above with a co-op / internship or other applicable work experience designation (if available and where applicable).If this option appears, your selected program of study offers a work experience designation (co-op/internship or other). Select "Yes" if you wish to pursue a work experience option or select "No" if you are not interested. If you select "No", you may still be able to participate in a Co-op or Internship work term (if available and if applicable requirements are met).

Student's EmailThe student will establish a single email address for communication with the College.
If you do not have an email address to provide, one can be provided to you by the college. You are expected to check your email messages regularly for admissions and registration related information.

Country of ResidencyThe country where you are legally residing at the time of application. If you are in Canada with a valid immigration status, please write "Canada".
Please enter today's date and sign the form. The student's signature MUST match the signature on the passport.

First Semester Ƶ Student Refund / Deferral Procedure

This form contains important Refund/Deferral information and deadlines. If you miss these deadlines, you may incur monetary and/or other penalties. Please retain a copy of this document for future reference.

Use of an authorized Education Consultant (if applicable)

Ƶ gives no preference to applications submitted directly or via education consultants.

Use this optional form if you want your Ƶ-authorized education consultant to submit your application to Ƶ for you. Your Education Consultant will email the completed form to Ƶ Admissions together with your additional application supporting documents.

To activate the form, check the box "I am appointing a Ƶ-authorized education consultant to submit an application on my behalf.

If applicable, fill out the form and forward it to your Ƶ-authorized education consultant.
You may check with us if your education consultant is authorized to represent Ƶ College.

Before you receive an Official Letter of Acceptance, if you have any admissions questions, please connect with your appointed education consultant.

"Draw a Signature" / Electronic Signatures

Ƶ continues to seek out ways in which it can reduce the need to print by creating "paperless" process.

Feel free to use the Draw a Signature function in suite, as long as the signature is in handwriting and matches the one in your passport. Do not insert scanned images of signatures.
